Checklist: before the appointment

Have you brought all of the important documents for your visit? You can check this with our practical checklist:

• Please fill out our medical history form before your first appointment, and bring it with you when you visit us. If you cannot print it out at home, we will have it available for you in our surgery.
• Electronic health card (for people with statutory health insurance).
• X-ray record card (if necessary). If you don't have one, we'll be happy to issue one for you.
• Allergy record card if necessary.
• If you have to take medication regularly: a medication list.
